Student of the Month Nomination Form

Grizzly Athletics believes in the power of positive role models and the importance of being involved in the community. Each month we would be proud to recognize elementary school students throughout the Missoula community.
Teachers and principals may nominate students they feel deserve the special recognition. Once a month Grizzly Athletics will review the nominations and select one lucky recipient for the "Student of the Month" award. As part of the award, Monte and a couple of Griz Student-Athletes will visit the classroom and present the award to the winner.
Rules:
- Due to travel regulations, the school must be within Missoula city limits.
- Nominations must be from teachers or principals only
- Nominations must be for kindergarten- 5th grade only
- Nominations may be for any reason the nominating person feels is worthy of being recognized. Please describe in the form.
- Nominees must be submitted by the 10th of each month. Winners will be selected and the nominating person will be contacted to by the 15th of each month to schedule a presentation before the end of the month.
- In the event your nominee is selected, Grizzly Athletics will ask for permission to use the student's photo on www.gogriz.com and all Grizzly Athletics social media outlets.
